Form Instructions:

  • Travel, Housing and Meals are the responsibility of the attendees.
  • Names, email address and contact information are required. Please insure the accuracy of the email address so we can more easily contact you. You will receive a confirmation email of this transaction.
  • Call our office if you have no email address or have questions. (512) 452-8700.
  • Children under 12 must be accompanied by their parent or lawful guardian.
  • Child's age is rounded down:
    18 mos = 1 year and 7 mos = 0 years

Fees:

  • Conference
    • 12 and over $45.00 per person
    • 4-11 $35.00 per child
    • 0-3 FREE Stays With Parent
  • Discount 10% of total due if paid before 05/15/2010 (Postmark)
